You may appear before the FACULTY/STAFF TRAFFIC APPEALS COMMITTEE at its next meeting to address these charges.  The FACULTY/STAFF TRAFFIC APPEALS COMMITTEE meets the last Thursday of each month from 1:30 p.m. to 2:30 p.m. in room 131 in the Parking Services office at 120 Zach Curlin in the front of the Parking Garage.

If you wish to appear in person, please indicate by circling YES below and return this form to the Parking Office at 120 Zach Curlin Parking Garage. If you wish to appeal, but cannot appear in person, please indicate by circling NO below and state your appeal at the bottom of this form. Please return this form to Parking Services. Failure to appear may result in the Committee upholding the charges against you.

If you do not appeal or your appeal is denied, payment is due at the Office of the Bursar within (10) working days of the Committee hearing. The amount of the citation(s) will be deducted from funds, including but not limited to salary, due you.
