Annual Enrollment Transfer Period

It is time to review your benefits and make changes for 2010. October 15 through November 16 is the Annual Enrollment Transfer Period (AETP). There have been significant changes to some of the insurance plans along with premium changes. Explanations of the changes can be found in the AETP newsletter PDF and other information you will receive at your home and campus addresses.

Handbooks and Directories

Directories: Each of the health and dental plans have their provider network directories available online. These lists are more up to date than their printed directories. Please follow the links through each of their Web pages to use the online directories. Copies can also be picked up from the Employee Benefits Office, 167 Administration Bldg.

Health Insurance: Transfer, Cancel
  • PCP form required for CIGNA HMO, and must be submitted with enrollment/change form.
  • If you are canceling your health insurance for you or any dependents, remember there are no open enrollment periods for health insurance.
Dental Insurance: Add, Transfer, Cancel Primary care dentist form required for Prepaid plan, and must be submitted with enrollment/change form.

Optional Term and Universal Life Insurance:
Increase, Decrease amount of coverage, Request coverage, Cancel coverage

All forms must be submitted to Employee Benefits.

If eligible for $5000 guaranteed increase, UNUM Provident will mail applications and new premiums to your home. Must return form to Employee Benefits.

Optional Special Accidental Death and Dismemberment: Add, Cancel Guaranteed coverage; no health evaluation needed.
Flexible Benefits: Waive tax-shelter of medical and/or dental premium, add dependent care and/or medical reimbursement accounts Must make these elections every year.
Long-term Disability: Add, Change Level Must complete health evaluation to add or increase level.
Supplemental Retirement Accounts: Enroll, Change, Cancel Can enroll and make changes anytime of year; not subject to annual enrollment period.
Sick Leave Bank: Enroll Can enroll anytime of year; not subject to annual enrollment period.
