If you did not enroll as a new employee, you may do so during the Annual Enrollment Transfer Period (October 15 - November 15) without proving insurability. Dependents may also be added during this time. All changes will be effective January 1. You may cancel at any time.

Description of Plan
The optional special accidental death insurance is in addition to the life insurance included in the group insurance plan and is underwritten by Fort Dearborn Life Insurance Company. The coverage amount is dependent on the employee's annual salary. Please see the Fort Dearborn Life Insurance Company handbook

Cost
The cost is dependent on the employee's annual salary.
