Optional Special Accidental Death and Dismemberment

Enrollment/Change Periods
If you did not enroll as a new employee, you may do so during the Annual Enrollment Transfer Period (October 15 - November 15) without proving insurability. Dependents may also be added during this time. All changes will be effective January 1. You may cancel at any time.

Eligibility
Employee eligibility:

  • Full-time employees regularly scheduled to work a minimum of 30 hours a week
  • Faculty employed a minimum of 30 hours a week for the full academic year
  • Part-time employees with 24 months of service regularly working 1450 hours per year

Dependent eligibility:

  • Legally married spouse as defined under TN State law
  • Natural or adopted children
  • Step-children, if you or your spouse has legal or joint custody or shared parenting
  • Children living in your home for whom you are legal guardian
  • Any dependent child claimed as a dependent on your federal income taxes and living in your home
  • Unmarried dependent children are eligible for coverage through the last day of the month of their 19th birthday
  • Dependent children between the ages of 19 and 24 must be claimed on your income tax or be a full-time student
  • Proof of a dependent’s eligibility is required. Please see the Definitions and Required Documents PDF list for acceptable proof

Description of Plan
The optional special accidental death insurance is in addition to the life insurance included in the group insurance plan and is underwritten by Fort Dearborn Life Insurance Company. The coverage amount is dependent on the employee's annual salary. Please see the Fort Dearborn Life Insurance Company handbook PDF for complete coverage amount.

Cost
The cost is dependent on the employee's annual salary.
